As soon as you enroll in CareConnect, you’re given a dedicated team to guide you and your family through disease-specific education, health insurance information, and financial assistance programs for which you may be eligible.

You can enroll in CareConnect Personalized Support Services by clicking on the button below. Please note you will be asked to confirm which Sanofi product pertains to your condition as either current treatment or potential future treatment.
Patient representatives can also complete this enrollment form on behalf of the patient.
If Spanish is your preferred language, there is an option to switch the language on the enrollment form from English to Spanish. Look for the toggle button in the top righthand corner of the form.

- You are diagnosed but not currently on treatment
- You are unsure which Sanofi product to select
- You would like access to a paper enrollment form

What is CareConnect Personalized Support Services?
CareConnect Personalized Support Services is an individualized support program sponsored by Sanofi for people with certain rare diseases and their families. It offers a dedicated and experienced team, personalized disease education, information about treatment options, and resources that can help eligible patients with access to treatment. Who can participate in this program?
CareConnect offers support for eligible patients living with certain rare diseases and their families. Participation is available to individuals whether they are or are not on a Sanofi treatment.
Is there a cost to enroll or participate in the program?
Participating in CareConnect is free, voluntary, and confidential. There are no fees to enroll or participate in the support program.
Do I need to be on treatment to enroll?
No, you do not need to be on treatment to enroll in CareConnect Personalized Support Services. If you are not on treatment and would like to enroll in the support program, you can contact CareConnect for assistance. A Case Manager is available to speak with you Monday through Friday, 8AM-6PM EST.

Do I need to be diagnosed to enroll?
No, you do not need to be diagnosed to enroll in CareConnect Personalized Support Services. If you suspect that you have a lysosomal storage disorder (LSD) and would like to enroll in the support program, you can contact CareConnect for assistance. A Case Manager is available to speak with you Monday through Friday, 8AM-6PM EST.
