At CareConnect Personalized Support Services, we understand that everyone has their own way of managing a rare disease. Our goal is to empower you with education and resources to help you find—and get—the support you need.
We can assist you in navigating your health insurance and available financial assistance if you’re eligible.
We’ll work closely with you to help you advocate for your needs as a patient or caregiver and coordinate with your care teams to encourage communication and support tailored to you. Our experienced team knows when to listen and when to help you prepare yourself to take the next step forward in your journey.
Your own dedicated team is made up of two dynamic experts in the healthcare space, a Case Manager and a Patient Education Liaison (PEL).
Case Managers
Case Managers have deep experience in the healthcare system and can work closely with you and your family to provide care coordination through transitions and help you understand insurance options and changes, financial assistance if you’re eligible, and how to handle out-of-pocket costs related to treatment.
Patient Education Liaisons (PELs)
Patient Education Liaisons (PELs) have clinical backgrounds in nursing or genetic counseling and are locally based in your area. They can provide in-person disease and treatment education for you, your family, and others in your community if you wish. For example, they can educate home health agency nurses on home infusions. They can also explain how the disease may be passed on in your family and complete a medical family tree by request.
