Individualized support for your patients living with a rare disease
CareConnect Personalized Support Services is a free, voluntary, and confidential program for eligible patients and families living with certain rare diseases.
What to know about CareConnect
Participation in CareConnect is available to all individuals who have or suspect they have a lysosomal storage disorder (LSD) such as ASMD (acid sphingomyelinase deficiency), Fabry, Gaucher type 1, MPS I (mucopolysaccharidosis I), and Pompe disease, whether they are on a Sanofi treatment or not on treatment. Your patients’ families and caregivers may also participate.
As soon as your patients enroll, they’re given an experienced team dedicated to helping them with questions or challenges along their unique journey.
Does your patient have a specific need or question?
We will work with you to support your eligible patients with services tailored to their needs.
Patients can talk to their Case Manager about:
• Health insurance
• Financial assistance
• Care coordination
• Assistance through life transitions (such as traveling, moving, or changing insurance)
Patients can talk to their Patient Education Liaison (PEL) about:
• Disease & treatment education
• Inheritance information
• Assessments & testing
• Locating a healthcare provider to discuss testing
When requested, CareConnect can provide in-person and telephone interpretation services in Spanish for patient support services.

One-to-one conversations with a Case Manager, programs, and materials to help your patients navigate insurance and, in some cases, assistance for those eligible.
Information about inheritance and genetic testing
Meetings with a Patient Education Liaison (PEL) for individual or family education about how certain rare diseases can run in families and types of genetic testing available.
Personalized disease and treatment education
Patient-friendly information for your patients, their family, and others in their community with in-person, phone, and virtual meeting options.
Individualized support for your patient at any life stage

We can provide information on medical family trees, genetic testing, and diagnostic labs.

We can assist with insurance verification and help your patient understand their coverage.

We can help your patient find treatment centers and resources related to home infusions, moves, vacations, and other logistical challenges related to their care.
Managing insurance changes?
We can help your patient navigate insurance changes related to employment, marital/dependency status, or other eligibility matters.

How can you help patients enroll?
Enrolling in CareConnect will help your patients be quickly connected to a team and resources tailored to their needs. Once you’ve diagnosed them or prescribed their Sanofi treatment, you can help them enroll in a few ways.
Send patients a link for digital enrollment
The button below gives you the choice to send patients an automated text or email with a link to a digital consent form that immediately enrolls them upon their electronic signature.
Connect patients to a Case Manager
When you direct patients to CareConnect, they can request to receive a link to the digital consent form or a printed consent form. Case Managers are available Monday through Friday, 8AM-6PM EST.

Empower patients with knowledge
By guiding patients to the CareConnect website, you’re helping them find the resources that matter to them. They’ll have the option to educate themselves about the program and enroll when they’re ready.
Financial programs designed for your patients
If your patients need financial assistance to access a Sanofi treatment for their rare disease, we may be able to help. CareConnect offers programs that may be able to help eligible patients manage out-of-pocket costs related to their therapy if they cannot access treatment through their health plan or do not have healthcare coverage.
Streamline the process for eligible patients
You can help streamline the application process for eligible patients by sending them a link to a digital consent form that immediately enrolls them upon their electronic signature.
CareConnect Copay Program
The CareConnect Copay Program* helps eligible patients in the U.S. who are prescribed certain Sanofi therapies pay for eligible out-of-pocket drug costs and certain infusion-related charges, including copays, coinsurance, and deductibles, up to the program maximum.
*Not valid for prescriptions paid, in whole or in part, by Medicaid, Medicare, VA, DOD, TRICARE, or other federal or state programs including any state pharmaceutical assistance programs. No claim for reimbursement of any out-of-pocket costs covered by the Copay Program may be submitted to any third-party payer, whether public or private, including but not limited to patients’ insurance, Flexible Spending Account (FSA), Health Savings Account (HSA), or any other type of medical savings account. This program is not valid where prohibited by law, taxed, or restricted. Sanofi reserves the right to rescind, revoke, terminate, or amend this offer, eligibility, and terms of use at any time without notice. Any savings provided by the program may vary depending on patients’ out-of-pocket costs. Patients will receive all program details upon registration.
Patient Assistance Program
The CareConnect Patient Assistance Program was established in the U.S. to provide certain Sanofi therapies at no cost to eligible patients who do not have health insurance or cannot access an eligible product under the terms of their insurance plan. This program is considered a temporary solution until coverage can be secured.
Additional assistance programs
CareConnect offers additional programs designed to solve obstacles eligible patients may encounter while accessing and maintaining therapy.
Product support services for you
Sanofi’s Product Services team provides dedicated Product Account Managers to assist with your purchasing needs for all Rare Disease Products.
Our Product Services team can assist with:
- Shipping and monitoring your orders to ensure accuracy, address delivery delays, and provide tracking information
- Providing infusion/refill reminders (where appropriate) in advance of a patient’s infusion/refill to help coordinate timely and accurate delivery of each shipment
- Returns, invoices, purchase history, and billing information
- Providing after-hours and holiday on-call emergency services
