After contacting our CareConnectPSS team, you will be connected with a dedicated Case Manager and receive a patient authorization form that provides information on:
- The full scope of our support services
- Confidentiality and personalized communication
CareConnectPSS support services are voluntary and confidential
FINANCIAL ASSISTANCE WITH THE CARECONNECTPSS COPAY ASSISTANCE PROGRAM
- This program helps eligible US individuals, who are prescribed one of Sanofi Genzyme’s treatments, pay for their eligible, out-of-pocket, drug-related expenses, including copays, coinsurance, and deductibles.
- To be eligible, you must meet certain conditions and must not have coverage or prescriptions paid for in part or full under any state or federally funded healthcare program
- Your CareConnectPSS Case Manager can provide more information about the Copay Assistance Program and the application process

Determine coverage and assist you in securing required authorization and referrals

Insurance verification and coordination when moving permanently or temporarily (eg, seasonally, college)

YOUR PATIENT EDUCATION LIAISON CAN HELP WITH:
DISEASE EDUCATION
Coordinates and leads telephone or face-to-face discussions that provide educational information and explain the disease in a way that makes it easy to understand: what to expect, stages of progression, and unique challenges
MEETINGS WITH YOUR SUPPORT COMMUNITY
Helps coordinate meetings with school or college teachers, coworkers, supervisors, or other members of your support community to help them better understand your diagnosis and the challenges you face
- These meetings would only be held at your request and with your permission
UNDERSTANDING INHERITANCE
Provides information on disease inheritance with tools, such as a medical family tree, to help identify additional family members who may be at risk for a lysosomal storage disease
